Here is a template email you can use to reach out to a referral source.

I’m writing to introduce you to Mammha, which is poised to begin offering comprehensive perinatal mental health services in [STATE] by the end of October. 

Mammha is free for use by healthcare providers and other perinatal professionals. It includes a provider portal featuring validated screening and risk assessment, screening reports and scripts, and other resources. In addition to care coordination, resource provision, psychotherapy and medication management, Mammha also offers a mobile app for perinatal patients and their families.

    There are 5 ways a Doctor, Doula, Hospital, or Health System can refer to Mammha.

    1. Use the Mammha referral form on our website.

    2. Use the Mammha QR code that takes a patient to the screen page. Every office or independent referral source get’s their own screening portal.

    3. Use the Mammha screenlink that takes a patient to the screen page. Every office or independent referral source get’s their own screening portal.

    4. Share the Patient Information sheet with the patient to initiate a self-referral. Every office or independent referral source get’s their own screening portal.

    5. Use the Mammha straight referral QR code or link. If a practice or healthsystem already has an established way to screen, referral sources can use a straight referral link. A straight referral link is collects their name, email, phone number, and if they are pregnant, postpartum, have experienced a loss, or are non perinatal.

    After a Positive Screen

Once Mammha receives a referral, Mammha will update the patient caseload tracker. Care teams and providers can review patient progress in the caseload tracker. Mammha can also reach out to providers to let them know there are updates on their patients in the caseload tracker.
