There are 4 ways MaineHealth providers could use Mammha to screen and refer.

  1. Use the Mammha referral form online (link below)

  2. Use the Mammha QR code that takes a patient to the screen page

  3. Share the Patient Information sheet with the patient to initiate a screen (Soon - a self-referral)

  4. Use the Mammha screenlink that takes a patient to the screen page

 
 

Patient Updates

Once Mammha receives a referral, Mammha will update the patient caseload tracker. Care teams and providers can review patient progress in the caseload tracker. Mammha can also reach out to providers to let them know there are updates on their patients in the caseload tracker.

If a patient calls to share they are having a hard time and need support or a referral. A provider can text or email back the following messaging:

More Streamlined Referral Coming SOON!

We are adding a new referral entry point for patients soon. This will allow patients to use the QR code to get to the Mammha screen, and it will give them the option to bypass the screen to enter into care. Once this is up and running we will let you know. We feel this will help providers to seamlessly refer patients without having them take the EPDS screen a second time to enter our system of care.
