Mammha Initial Contact Checklist
Goals: To let her know she is not alone, and that you are there for her. To learn as much as you can to be able to connect her to all the right resources.
Remember, just the act of connecting with her makes her feel less alone and more hopeful.
Reach out within 24 hours of mom screening positive–preferred response time is within 1 hour
Introduce yourself via text from your Care Coordinator Portal. Ask her what her communication preference is.
Once contact has been made, ask her how she is feeling.
If she shares what she is feeling, listen to determine if there is any way you can help – getting better or more sleep, managing anxiety, or worry thoughts, dealing with boundaries, communication between couples, loneliness, panic attacks, feeding the baby
If she does not respond back with a lot of detail, ask open-ended questions: How are you sleeping? How are you eating? How is your support system? How was the birth?
Ask her if she would like to be connected to a therapist, let her know you can help find her one who specializes in working with moms, and also accepts her insurance.
Make sure she knows you are there, and will help connect her to the best support.
If she says yes to wanting a therapist, be sure to inquire if she wants the therapist to speak a specific language or come from a certain cultural background.
Inform her that you will contact therapists to find her a good match. Let her know that you will always check back in with her once you connect her with a therapist to make sure it feels like a good fit.
Let her know if there is a speciality support group coming up that day or later that week.
Share any articles or podcasts, or any resource that can help her based on your previous conversations.
Make sure she knows you will reach back out to her.
Assure her she is not alone.
